Immigration Medical Examinations (Form I-693) performed by a USCIS-designated Civil Surgeon. $439, covering two office visits; payment is due at the first visit. Process: (1) initial exam, which identifies any additional testing needed; (2) insurance-covered additional testing at a facility of the patient's choice, with referrals provided; (3) a second visit to complete and seal Form I-693; (4) the patient receives the sealed documents to submit with their immigration application. Bring an unsigned, most-recent-edition Form I-693 with Part 1 completed (but not signed), medical and vaccination records, and a photo ID. Language translation is available during the visit.
